I have been unsure whether I should input my name with my surname first? Or first and then surname? The form phrases it as "Please input the surname and first name on your valid certificate" which is why I thought it might be surname first. I have mine with the surname last on my HSK certificates (whereas all my bank accounts etc in China have my surname first).
