Are these all correct ways to ask, "Have you ever been to Taiwan?"

Some of them are more awkward than the others, but are they all grammatically correct?

If you're really fond of the positive-negative structure, it is grammatically correct to say

I think the first one sounds okay, but not as smooth as the 吗 structure. The second one sounds quite awkward (to my non-native ears).

How about asking if someone is hungry?

Perhaps 有沒有餓 is grammatically inappropriate. However, orally, we do say (influenced by Taiwanese, I guess) or 是不是餓了啊/呢? Also, you can say

and.....beautiful?

Once again, 3. is orally used.

Answers suggested by renzhe and yonglin are correct, too.
