It depends--are you sending it from abroad into China? You can start with smallest element (the person) and work up to the largest element (country) and it can be done line by line in English:

 

Zhu Jianhong

Shangde Building, Block A, Room 2313

Wushang Road, No. 141

Tianhe District, Guangzhou, Guangdong 510630

CHINA

 

If you are sending it from within China, do the opposite and go from largest (province) to smallest (person) element, written in one (or a few) long string(s) of Chinese:

 

Guangdong Province Guangzhou City Tianhe District Wushang Road 141 No. Shangde Building A Block 2313 No. Zhu Jianhong

510630

If you're sending a package it's common to include the telephone number.

Place it below everything else. And identify it as a phone number. (Important do do this. Half the time the delivery people cannot find a residential address and will need to call the recipient for directions.)

 

For example:
