American names changed to Chinese

I'm referring to the city names, as well as famous stars. Who creates these? And what language is it in? Once it is published in a paper, are all others obligated to use it?

For instance, I'm Cantonese, so when I read the paper (okay, I can't read, but when people translate to me), the names of American cities and actors are literal translations, such as "Nau Yeuk" for New York, and "Ah No" for Arnold. When translated into Mandarin, sometimes these words make absolutely no sense.

Is that why many Mandarin people can't read Cantonese newspapers and so on and so forth?
