terms relating to massage, accupuncture and appointments

Hi, I need to learn some new terms and phrases for a potential new job as a receptionist in a chinese natural therapy health centre. My chinese is intermediate but i'd just like to make sure I am saying these things correctly (I don't need pinyin, simplified char's are easier for me but either is ok).

I need:

* terms relating to chinese massage / accupuncture / cupping / reflexology / remedial massage / aromatherapy / naturopathy etc...

* terms such as practitioner, treatment, health insurance, booking

* body parts - head, neck, back, shoulders, whole body...

* Common health problems, eg, arthritis, muscular aches and pains, carpel tunnel, insomnia, poor circulation, sports injuries.

* phrases relating to appointments, eg, "do you have an appointment?", "could you please fill out this form for our records", "would you like to schedule your next appointment?", "how long a treatment would you like", "how would you like to pay - cash, credit or savings?" etc... and this sentence: "After the tenth treatment we will offer you one treatment for free (at an average cost of the previous 10 treatments)"

I will probably think of more stuff i need to know so ill just add as i think of them...

of course i have only put in examples but anything you think could help would be greatly appreciated...
