I think its possible English translation may be “achieve certain goals through some kinds of hardworking”. No. How my pains can be described. 用心良苦 has two parts: 用心 -> intentions, efforts 良苦 -> 良(good) 苦(苦心) The whole thing means "good intentions/caring (that might or might not have produced good results.)" " won't sound right in English, because English grammar almost always requires the subject (which is unlike Chinese, Japanese, Vietnamese, etc.) You know, even when there's no subject, you have to invent one: It rains. So the whole word has nothing to do with that the intention is good or not, and it is simply used to describe someone who pays much thought to some matter. There're other words which have the similar meaning with nuances. 深谋远虑/深思熟虑 is commendatory (褒义词), 处心积虑 is derogatory (贬义词), while 用心良苦 is neutral (中性词).
